Remove the unrequested Consumption checkbox

Remove the Include hidden and special areas checkbox, its state and unused styles. Every website report request continues to apply the approved area exclusions when loading, changing levels or paging. Update the existing interaction test and reporting documentation to describe the fixed exclusions. Follows !80 (merged); related to #22 (closed).

Validation: all 22 frontend tests and the production frontend build pass (existing unrelated warnings remain). Rendered desktop/mobile checks show no checkbox, preserved filtered totals, working pagination and level submission, and no mobile page overflow. Backend/report data logic is unchanged.
